Our Flight Request Process
Children’s Flight of Hope is focused on serving as many children as possible. To ensure our resources are used well, we assess each family’s medical and financial need for assistance. Families complete CFOH’s Program Application with information about their child’s treatment needs and key medical and financial contacts. Requirements for acceptance are listed below. Application review typically takes 3-5 business days.
After an application is approved, flight requests can be submitted for the child’s travel to medical appointments or treatment as needed moving forward. The CFOH Missions Team works directly with client families to choose flights and provide reservation information. CFOH strives to assist with flights for client families who maintain eligibility throughout the duration of a child’s treatment needs.
Medical Need— CFOH services are provided to access treatment that may save, prolong or enhance the quality of life for the child. Flights will only be booked for a confirmed medical appointments or scheduled treatment. CFOH confirms appointments, medical need and stability for air travel with referring and/or destination physicians.
Financial Need— Services are provided when the costs of air transportation would prohibit travel or place significant financial hardship on the family requesting assistance. Acceptable verification of financial need includes proof of government subsidy or completion of a CFOH form by the child’s social worker or other relevant professional.
Timing— Potential clients must apply to and be accepted for CFOH assistance before submitting a flight request. Approval for program assistance typically takes 3-5 business days. Once a family has been approved as a client, they can submit flights requests moving forward. Flights must be requested at least two weeks before the travel departure date.
Age— Flights are only provided to children under age 18 and one adult travel companion (typically a parent/guardian).
Service Area— Client families must be traveling to medical facilities in the United States. Flight requests originating internationally must be referred to CFOH through the American Airlines Well-Being program or other partnering organizations.

How to Request a Flight
If you are an existing CFOH family (domestic or international), please use the returning client form here to complete your request. You will need the child’s client ID and date of birth to begin the application.
CFOH is evaluating new family requests and accepting based on current resources. If you’re interested in flight assistance, please complete the new client application here for consideration.
Private Flight Requests
CFOH arranges private air transportation only when medically necessary and when possible with available resources. Currently, we are accepting private flight requests on a very limited basis and within 600 flight miles. Our friends at AeroAngel may also be able to help with private flight needs.
