How to Request a Flight

Returning Families

If you are an existing CFOH family (domestic or international), please complete the Returning Families Flight Request form. You will need the child’s client ID and date of birth to begin the flight request.

New Families

CFOH is welcoming new families at this time. If your child meets the eligibility requirements below, please complete the New Families Application.

Private Flight Requests

CFOH arranges private air transportation on a limited basis when verified medically necessary. CFOH is committed to providing flights for the duration of a child’s treatment needs.

Our Flight Request Process

Children’s Flight of Hope is focused on serving as many children as possible.

To ensure our resources are used well, we assess each family’s medical and financial need for assistance. Families complete CFOH’s New Client Application with information about their child’s treatment needs and key medical and financial contacts. Requirements for acceptance are listed below.

After an application is approved, flight requests can be submitted for the child’s travel to medical appointments or treatment as needed moving forward. The CFOH Missions Team works directly with client families to choose flights and provide reservation information. CFOH commits to helping with flights for the duration of a child’s treatment needs.

Our Requirements

Medical Need

We provide flights to life-saving, life-changing and life-sustaining health care. Flights will only be booked for confirmed medical appointments or scheduled treatment. CFOH confirms appointments, medical need and stability for air travel with a child’s referring physician and/or specialist. 

Financial Need

We assist when flight costs would prohibit travel or place financial hardship on the requesting family. To verify a family’s financial need, we request that the child’s social worker (or other relevant professional) complete our financial verification form.  Alternatively, a family can provide evidence of government subsidy (e.g., Medicaid, Disability).

Timing

Families must submit a New Client Application and be accepted to our program before specific flight requests can be submitted. Once a family has been welcomed to our program, they can submit flight requests moving forward. Ideally, flight requests should be submitted at least 2 weeks before the travel departure date.

Age

Flights are provided to children through the age of 18 and one adult travel companion (typically a parent/guardian).

Service Area

Client families must be traveling to medical facilities within the United States. If an international child is traveling to the US for treatment, we accommodate their request through our partnership with the American Airlines “Miles for Global Health and Well-Being” program.
